Medical Form

To expedite your visit please download the Happy KiDDS medical form and fill out prior to your first visit. If you have any questions about the form, just contact our office and we will be glad to help you. We look forward to working with you and maintaining your child‘s dental health.

Authorization for Treatment and Payment

If someone other than a parent or legal guardian will be bringing your child for their dental appointment, please call ahead to provide authorization for treatment and to arrange for payment.

Running Late

Happy KiDDS understands your time is valuable and makes every effort to remain on schedule. If you are stuck in traffic, running late or just can’t seem to get here on time, please call ahead so we can do our best to accommodate you. If you are going to be more than 15 minutes late, your appointment may need to be rescheduled in order to ensure the next patient receives his/her full appointment time.

Cancellations

Happy KiDDS requires at least 24 hours notice for a cancellation. Should you fail to cancel, a $100 fee will be charged to the account.
